Advanced Illness

Our Advanced Illness Case Management team assesses a participant's condition status, and offers contact from a specially trained registered nurse.

Participants may then receive support services, including:

  • Guidance and assistance by phone,covering a wide range of medical, psychosocial, and practical issues
  • Educational resources, including a question guide
  • Coordination of health care services, to facilitate communication when the site of care changes, and to assist the member in accessing resources
